B. Nasal fractures and septal hematoma

B1. Anatomy

(1) Frontal bone, the frontal process of maxilla, the perpendicular plate of the ethmoid

(2) Cartilage : Nasal septum, lateral processes, medial & lateral crura of the alar cartilages

B2. Clinical features

(1) Injury의 mechanism 이 중요

(2) Assess the midface, zygomatic arch, orbits, sinuses, teeth, and cervical spine

(3) Naso-orbital-ethmoid 손상은 아래 그림처럼 나뉜다 (Fig 244-9 – 8판 그림)

(4) Malocclusion and palatal instability → Le Fort fracture (Ch 255에 설명)

B3. Nasal exam

(1) Bony crepitus, deformity, edema, periorbital ecchymosis, profuse epistaxis 확인

(2) Orbital injury에 다른 증거가 없으나 periorbital에 ecchymosis가 있다? → Nasal fx. 가능성

(3) 다량의 코피 → Nasal fx. 가능성

(4) Internal examination

: Mucosal lacerations, Septal fractures or deviation, Septal hematoma를 꼭 확인

B4. Imaging

(1) 골절 진단은 임상적으로 한다 (2) 방사선 검사가 꼭 요구되지 않음 (3) Plain film의 결과는 치료 계획을 바꿀 수 없음 (4) Closed reduction은 nasal obstruction이나 cosmesis를 향상시키는데 제한적이며 보통 응급 시술이

아니다 (5) US : Plain film과 sensitivities and specificities가 비슷하다. (6) CT는 isolated nasal Fx.에서는 시행하지 않음.

: Intracranial injury or other facial fracture 등 타 injury 감별 위하여 시행

B5. Treatment

(1) 다른 traumatic injury (Ch 255에 자세한 설명) 와 nasal septal hematoma를 배제하는 것이 중요 (2) 열상과 동반된 비골 골절은 open fracture와 동급으로 치료 받아야 한다. (3) CSF rhinorrhea or otorrhea → Imaging and otolaryngologic, neurosurgical consulation (4) Children의 nasal bone fx. 는 2~4일 내로 ENT로 refer (Rapid healing 되기 때문에) - 9판 삭제 (5) 시간이 지날수록 edema, swelling으로 검진이 어려워 짐. - 빨리 검진할수록 좋음

(6) 사고 발생 6~10일 안에 ENT consultation & elective closed reduction

: 기한을 넘기면 rhinoseptoplasty가 필요할 수도 있다. (7) 대부분 nasal fx. 는 즉각적인 intervention이 필요하지 않음 (8) Closed reduction은 ENT에게 맡기자

B6. Nasal septal hematoma

(1) 발견 즉시 septum의 괴사를 막기 위해 incision & drainage → Saddle deformity, septal abscess, osteomyelitis, cavernous sinus thrombosis, meningitis,

intracranial abscess 등으로 진행 가능하므로 초기의 빠른 치료가 중요 (2) Facial trauma 후 systemic sx → Abscess 의심

→ ENT refer, urgent surgical drainage, IV ABx.

(3) Drainage of nasal septal hematoma

: Sniffing position → Anesthetize nasal mucosa → 완전한 aseptic 환경 아니더라도 최대한 clean 하에 환경 조성 → Horizontal incision & suction → Frazier suction or with forceps으로 clot 제거한다 → Bilateral ant. packing with nasal tampon coated in topical antibiotics → Oral antibiotics & 24hr OPD f/u (24~36시간 안에 packing 제거 시 ABx. 필요 없음)
