(1) 정의
: Joint와 이를 둘러싸고 있는 anatomic structure에 통증을 일으킴
(2) Clinical features
•
주로 masticating muscle에 통증을 호소하거나 씹을 때 한쪽 또는 양쪽의 TMJ에 통증 호소
(3) Physical exam. findings
① Mandible 움직임의 제한
② Mastication muscle을 촉진하면서 민감해졌거나, 경화되거나, 단단해졌거나, 부은 곳을 확인
(4) DDx.
① Jaw trauma (fracture, dislocation)
② Odontogenic pain (abscess, carie, or trauma)
③ Otologic referred pain (otitis media, otitis externa, foreign body)
④ Temporal arthritis
(5) Diagnosis
① 병력과 신체검진에 의해 진단
② Acute trauma의 경우 panoramic x-ray view가 initial imaging choice
③ CT는 complex fracture, infection, neoplastic disease를 assess 하는데 적절
(6) Treatment
•
Uncomplecated fracture, nontraumatic condition의 경우에는 NSAID를 포함한 진통제를 주거나
필요시엔 마약성 진통제 고려
•
연식 먹도록 권장
•
chronic temporomandibular dysfunction의 경우 치과 협진
