압력에 의한 통증 및 bluish discoloration
Blood evacuation 으로만 통증 감소 가능
Trephination 방법
•
Heated paper clip : hematoma의 coagulation 유발 및 carbon partile 남김 : tattoing 및 회복지연
•
Neelde, scalpel, nail drill : 압력이 가해지므로 통증 유발
•
Hand-hel electrocautery : rapid and painelss
•
3mm diameter curette
•
3mm diameter punch
Evacuation은 6~12시간 내에 시행해야 함.
•
압력에 의한 Nail 재생 지연 및 nail bed healing 을 위해
B. Nail bed injury
Nail removal 은 partial nail avulsion 또는 surrounding nail fold disuption 이 있는 경우에만 권장
Repair with suture
: nail bed 열상이 있는 경우 6-0 흡수사를 사용하여 조심스럽게 봉합해야 함.
: hand surgen 은 대개 interrupted suture 시행함 (vicryl)
Repair with adhesives
: suture와 유사한 breaking strength를 보임
C. Replace the nail
Saline으로 부드럽게 세척
5-0 비흡수사로 봉합
원래 nail을 찾을 수 없는 경우, 대체 제품으로 공간메우기
D. Distal phalanx Fx.와 동반된 경우
50%의 손톱 손상이 distal pahlanx fx,가 발견된다.
Non-displaced distal tuft fx. - nail repair 와 nail plate replacement 로 치료.
Displaced Fx.와 nail
fold 보다 proximal part의 Fx.는 IF로 치료
E. Nail bed avulsion
Germinal matrix fragments는 보전되어야 한다.
가능하면 Free graft를 6-0또는 7-0 흡수사를 사용하여 봉합한다.
Nail bed 손상이 크거나 defect가 큰 경우, hand surgeon 이 필요함
