Routine Wrist X-Ray 를 ABC 계통적 평가법으로 평가할 수 있다.

A. AP View

A1. Adequacy

AP & Lateral view 필요
Scaphoid Fx.가 의심되면 Scaphoid view 필요
No deviation Lunate overlies the DRUJ

B. Alignment

(1) Three arches of the articular surfaces of the proximal and distal carpal rows

(2) Radiocarpal joint in neutral position
Without ulnar or radial deviation
3rd metacarpal shaft in line with the shaft of radius
Lunate straddles the DRUJ (distal radioulnar joint)
모든 관절강이 동일한지(1~2 mm), 평행한지
3개의 arch
Lunate가 사각형 모양인지
Scapho-lunate 간격 확인
원위 Radio-Ulna 간격 확인 - 겹치거나 거의 닿아 있어야

C. Bone

Carpal bones ---- scaphoid
Scaphoid ring sign – rotary subluxation (scapholunate dissociation)
Lunate – trapezoidal appearance
Proximal metacarpals, distal radius and ulna

D. Cartilage

Intercarpal joint spaces – uniform (2mm)
DRUJ – separated by 1 ~ 2mm

B. Lateral view

B1. Adequacy

(1) Dorsum surface of the ulna is 1 to 3mm dorsal to the radius (2) No flexion or extension at the wrist

B2. Alignment

(1) Sequence of adjacent Cs
– articular surfaces of the distal radius, lunate, capitate are aligned
(2) Axis of the scaphoid
(3) Ulnar styloid – point to dorsal surface of the triquetrum

C. Bones

Distal radius
Triquetrum (dorsal surface) – chip fracture

D. Soft tissue

Pronator quadratus fat stripe
Radius-Lunate-Carpitate-3rd metacarpal bone은 서로 관절을 이룸
True lateral position
Dorsum surface of the ulna located 1 to 3mm dorsal to the radius
Dorsal surfaces of metacarpals
parallel to the dorsal surface of radius

C. Pronation oblique view

C1. Adequacy
: trapezium and trapezoid joint space – minimal overlap
C1. Bones
Bases of I and II metacarpals; trapezium and trapezoid Scaphoid – distal pole Triquetrum Distal radius
Joint space between the trapezium and trapezoid is visible
Minimal overlap between proximal 1st and 2nd metacarpals
All view should include the proximal metacarpals
and the distal 5~6 cm of the radius and ulna

D. Sites prone to injury and easily missed injuries
