III. Clinical Features

1. Pulmonary Barotrauma is m/c fatal primary blast injury 2. Vent Care 시 Tidal Volume은 6 to 7mL/kg to IBW (ideal body wt)로 peak inspiratory

pressure를 제한하여 ventilator-induced lung barotrauma를 예방한다 3. 다른 증상이 없더라도 Tympanic memebrane perforation 이 확인된 경우 Chest X-ray를 시행해야

한다. (TM perforation은 primary blast injury의 indicator로 사용될 수 있고 최대 50%에서 lung injury의

진단을 놓칠 수 있다.) 4. Air는 poor energy conductor이므로 폭발 바로 근처인 경우가 아니면 복부 손상은 드물다. 5. Vascular injury 가능성이 있는 limb perfusion, pulse 등을 관찰한다.

특히 Compartment Syndrome 발생 가능성이 있는지 close observation한다. 6. Renal failure: most serious complication of crush syndromes
