1 Systemic vasculitis : carotid circulation에 있는 medium-sized and large a. 침범
2 대개 50세 이상. polymyalgia rheumatica를 가지고 있는 경우가 많음.
3 Painless ischemic optic neuropathy with profound visual loss를 유발할 수 있으며
즉시 치료하지 않을 시 반대쪽도 빨리 involve 할 수 있음( 수 일~수 주 내 반대측 눈 침범)
4 증상 : Anorexia, fever, fatigue, jaw claudication, temporal artery tenderness,
myalgia, headache, scalp tenderness, severe painless visual loss
33%는 TIA나 ischemic stroke 같은 신경학적 증상과 관련
5 P/Ex
: Palpably tender temporal artery or nodular or no pulse
: Fundus - pale disk swelling with nerve fiber hemorrhage on & near the disk
: Lab ESR (70~110mm/h), CRP 증가 (정상일 수도 있음)
6 치료
: Corticosteroid

Visual symptom 없으면 → Prednisone 60mg PO/day, 1주 이내 안과 f/u
•
Visual symptom이 있는 경우 → IV Methylprednoslone 5001000mg x 3day
: Temporal artery biopsy는 스테로이드 치료 후 1주 이내로 시행되어야 한다.
: Steroid에 효과없거나 못 쓰는 경우 Tocilizumab
