A. Vesicular breathing sound (폐포음)

약한 바람부는 듯 한 저음조의 소리
흡기음이 호기음보다 크고 길다 (정상)

This is the sound of vesicular breath sounds. They are considered normal, described as low pitched. B. Bronchial breathing sound (기관지음)

폐포음보다 거칠고 속이 빈 것 같은 소리 (large airway sound)
흡기와 호기의 길이는 동일하고, 호기 소리가 더 크다 (피치가 비슷하다는 의견도 있음)
Trachea 부위에서 들리면 정상, 말초에서 들리면 폐렴, 흉박삼출, 폐허탈 가능성 있음

C. Crackle (악설음/수포음)

폐포 및 소기도가 열리면서 나는 소리로, 폐포가 약체로 차 있을 때, ILD같은 간질의 섬유화, microatelectasis 때 들린다.

C1. Fine Crackle (aka, rale)

작은 소기포가 터지는 소리, 눈을 밟는 소리 등으로 표현
Fine crackle은 흡기/호기에 모두 들릴 수 있다.
흡기/호기 초기에 들리는 Fine crackle은 chronic bronchitis 의 전형적 청진소견이다.
흡기 말기에 들리는 fine crackle은 폐렴, CHF, 또는 atelectasis 를 시사한다.
폐하엽에서 잘 들린다.

Fine Crackles (aka Rales) are high pitched sounds mostly heard in the lower lung bases. C2. Coarse Crackle

낮은 피치의, 좀 더 크고 긴 악설음. bubbling sound라고 부르기도 함.
간헐적으로 열렸다 닫혔다 하는 기도에 공기가 통과할 때 나는 소리
흡기/호기 모두에서 들릴 수 있다.
Lung base에서 잘 들리는 fine crackle과 달리, 특별히 더 잘 들리는 부분은 없다
만성 기관지염, 기관지확장증, 폐렴, 심한 폐부종에서 들을 수 있다.
Coarse Crackles (Rales) - Lung Sounds - Medzcool
Coarse Crackles are low pitched lungs sounds heard in pathologies such as chronic bronchitis, bronchiectasis, pneumonia, and severe pulmonary edema. D. Rhonchi (건성 수포음)

삑삑거리는 소리. 천명음 중에서 진동음으로 들리는 것 (낮은 피치의 천명음)
코고는 듯한, 거글링, 럼블링으로 표현되는 소리 (터뷸런스)
대기도 쪽에서 더 잘 들린다 (가슴의 중앙부에서)
종종 대기도나 기관지에서 많은 분비물 때문에 들린다 (기침하거나 suction후 사라질 수도 있다)
COPD, cyctic fibrosis, bronchiectasis, pneumonia 등에서 들릴 수 있다.

This is the sound of rhonchi when auscultating breath or lung sounds. It is caused by thick secretions in large airways as air passes by. E. Wheezing (천명음)

좁아진 기도를 공기가 흐르면서 생기는 연속적인 소리
높은 피치
흡기 시에는 기도 내강이 넓어지므로 대부분의 천명음은 호기 시 뚜렷함.
고정된 기도협착이 있는 경우 흡기시에도 들릴 수 있다
단조성 : 기관지암, 기관지협착, 이물흡인
복조성 (polyphonic) : 기도 내강이 전반적으로 좁아져 있거나, 좁아진 정도가 균등하지 않을 경우 : 천식, COPD, bronchospasm, viral infection…
