B2. Examination
(1) Sequential examination
•
Visual acuity, confrontational visual fields, EOM, pupillary reaction, lids&adnexa,
conjunctiva, sclerae, cornea, ant. chamber, iris, lenses, vitreous, IOP (검사 후반부),
funduscopy (마지막). 모든 검사를 모든 환자에게 수행할 필요는 없다.
•
Visual acuity : Vital sign of the eye
Chemical burn 환자에서 Irrigation을 먼저 하는 경우를 제외하면 모든 안과 검사의 first step
절반 이상을 읽을 수 있는 가장 작은 줄이 시력이 된다.
•
Confrontational Visual Fields
•
Ocular motility
: CN IV - SO, CN VI - LR, All other EOM - CN III
: Diplopia 는 대개 이상있는 외안근 쪽을 응시할 때 더 심해짐
•
Pupil
: Relative afferent pupillary detect (RAPD), Marcus-Gunn pupil
: 손상된 쪽 눈에 빛을 비추면 동공이 축소되는 것이 아니라 확대된다.
→ Optic nerve disorder를 시사
•
External eye
: Periorbital Skin, Lids, and Adnexa
: Cotton application – 윗눈꺼풀만 확인 가능
: 확인을 위해 Retraction 걸 수 있고, Book Clip을 써도 된다.
•
Examine the Iris for Pupil Irregularity and Pupillary Dysfunction
•
Fluorescein Examination (플루오레신 검사)
: Seidal Test
→ 각막상피의 손상을 보기 위한 test, Fluorescein strip에 saline을 몇 방울 떨어뜨리고 환자의 inf. conjunctival fornix에 이를 갖다 댄 다음 환자에게 눈을 몇 번 깜빡이게 하여 dye가 잘 퍼지도록 한 뒤 Wood lamp나 cobalt-blue filter를 통한 빛 아래에서 cornea에 fluorescein-tinged aqueous humor의 stream이 보이는지(양성) 확인함.
: Corneal abrasion에서는 bright green으로 보임
•
Fundoscopic Examination (안저검사)
: Size, Shape, Sharpness of the borders of the optic disk, the cup-to-disk ratio, size ratio
of the arteries to veins (Normal 2:3)
•
Papilledema
: IICP에서 Optic nerve의 bilateral edema가 발생하는 것
: Malignant hypertension, pseudotumor cerebri, intracranial tumors, hydrocephalus 등에서 Common finding
•
Intraocular pressure
: Normal Inraocular pressure : 10~20mmHg
: Globe rupture가 의심될 때는 Intraocular pressure를 측정하면 안됨
•
Schiötz Tonometer
: Intraocular pressure를 읽는 게 아님. 부정확하고 환자에게 불편하다.
•
Tono-Pen

Applanation Tonometer
(2) Slit lamp examination
•
Anterior Chamber를 제외한 Conjuctiva, Sclerae, Cornea, Anterior chamber, Iris, Ciliary body 중 Ciliary body를 제외한 나머지 부위에 대하여 검사 가능
•
빛을 비추었을 때 가장 먼저 보이는 구조물이 Cornea (Abrasion, Ulcer, Edema, Foreign body 등 관찰) → Anterior chamber (WBC, RBC 등 관찰, Iritis, Hyphema 등)
→ Lense
