(1) Mild conjunctival injection or subconjunctival hemorrhage(abrasion), bleed(laceration)
(2) 다른 구조물이 involve 안된 경우에는 erythromycin ointment (2~3일), suture는 거의 요구되지
않음
(3) Foreign body의 경우 대게는 cotton-tipped application으로 제거 가능
A1. Corneal abrasion
(1) 증상
•
통증, 이물감, 눈물, Photophobia, lid swelling, blepharospasm/conjunctival injection
(2) 원인
•
Contact lens 착용, 손톱, makeup brush, foreign body
(3) 진단
•
Seidal test는 negative
•
Fluorescein stain 시 bright green으로 나타나는 superficial irregular corneal defect를 보임
(4) 치료 - 대부분은 자가 치료되나 통증 경감과 감염방지를 위한 치료를 시행함
•
Oral or topical NSAIDS, Topical Tetracaine (24시간 이내로만)
•
Patching
: Healing을 촉진시키지 못하나 일부 환자에서 심적으로 편안해 함.
: Finger nail, vegetable matter, 콘택트렌즈에 의한 것일 경우 감염 위험이 높아 patch 적용
•
Topical antibiotics
: Contact lens와 무관한 경우 → Erythromycin eye ointment (3~4회/일), (patching 해도 됨)
: Contact lens와 연관된 경우 → Ciprofloxacin, ofloxacin, tobramycin ointment (3~4회/일)
: Organic Source : Erythromycin eye ointment (3~4회/일)

Large abrasion이나 central visual axis에 있는 abrasion은 24시간 뒤에 안과의사에게 의뢰하고,
small abrasion의 경우 48~72시간 뒤에 안과의사에게 의뢰해도 된다.
A2. Corneal laceration
1 Full-thickness corneal laceration 시사소견
- misshapen iris, macro- or microhyphemia, visual acuity의 감소, shallow anterior chamber
2 Seidal test 양성이나 small laceration인 경우 spontaneously close 될 수 있어 음성으로도 나울
수 있음.
3 Penetrating injury 의심되면 orbit CT를 시행하여 globe anatomy를 확인 후 필요 시 안과 의뢰
A3. Corneal foreign body treatment
* 대게는 superficial, benign이나 penetrating foreign body의 경우 시력상실 할 수 있다.
* Gross hyphema, microhyphema가 보이면 globe perforation 시사.
1 Topical anesthetic(0.5% proparacaine) in both eyes 눈깜박임 방지
2 Superficial한 이물인 경우 NS로 먼저 irrigation 시행한 뒤 25-gauge needle(using needle
bevel-up) 이나 sterile foreign body spud on an Alger bush를 이용하여 이물을 dislodge
시키고 나서 moistened cotton applicator로 제거
3 이물이 단단하게 붙어있거나 cornea에 박혀있으면 slit lamp를 이용하여 penetration 깊이를
확인해야 하며, full-thickness corneal foreign body 시에는 안과의사가 제거해야 함
4 제거 후에는 topical antibiotics, cycloplegics, oral analgesics 처방하고 필요 시 tetanus
prophylaxis도 시행 후 귀가
5 Residual rust ring (metalic foreign body가 수 시간 이상 남아 있는 경우)이 있거나 이물이
central visual axis 상에 있다면 다음날 안과의사에게 f/u 시킨다.
6 Superficial rust만 ED에서 제거하며 이때는 superficial burr만 시행함. 만약 rust ring이
visual axis 상에 존재하면 scarring이 발생할 가능성이 높기 때문에 ED에서 시행하면 안 된다.
7 성공적으로 제거한 경우 topical antibiotics, cycloplegics, oral analgesics 처방 후 48시간 이상
지속 시 안과 f/u 권고
