CHAPTER 73. Injuries From Nonvenomous Aquatic Animals

A. Divisions and Definitions

those that bite, rip, puncture, or deliver an electric shock without envenomation

those that sting (envenom)

those that are poisonous on ingestion

those that induce allergies

Although many injuries and envenomations have unique clinical presentations, the cornerstone of therapy is immediate attention to the airway, breathing, and circulation

C. Wound Management

C1. Wound Irrigation

•

Normal Saline > tap water 권장. 바닷물은 권장하지 않는다.

•

최소 100~250cc 이상, 상처마다.

•

Wound 가 highly contaminated인 경우 Povidone-iodine과 같은 antiseptics를 사용 할 수 있다. 단, antiseptics를 사용한 경우 NS나 tap water로 추가 irrigation 해줘야 한다.

C2. Wound Debridement

•

Debridement is more effective than irrigation at removing bacteria and debris.

•

Anesthesia가 도움이 될 수 있다.

•

Surgical debridement가 필요할 때도 있다. (foreign body, teeth, spine 등의 localization을 위해)

C3. Wound Closure

•

The decision to close a wound must weigh the risk of infection.

•

Tight wound closure restricts drainage and promotes bacterial proliferation, particularly from anaerobes, which are common contaminants.

•

Whenever possible, the use of sutures to close dead space in contaminated wounds should be minimized because the absorbable sutures act as foreign bodies.

D. Bacteriology of the Aquatic Environment

•

동물의 등뼈, 조개 등 / 서핑, 스노클링, 다이빙, 낚시 등 / 청소되지 않은 어선, 기타 구조물 등 à 수생 환경에서 얻은 상처는 쉽게 감염되고, 표준 항균 요법에 반응하지 않을 수 있음 à 광범위 항생제의 사용이 도움이 된다.

D1. Marine Bacteriology

(1) Marine environment

•

주로 혐기성 미생물이 많다.

•

해수면 근처에 bacteria가 가장 많지만, 침전물, 모래, 해양 생물의 구강 내에도 존재한다.

•

상어는 많은 약물에 내성을 가진 bacteria를 품고 있다고 알려져 있다.

(2) Diversity of Organism

•

다양한 약물에 내성을 가진 경우가 많아서, 광범위 항생제를 사용하는 것이 좋다.

D2. Wound Infection Caused by Vibrio Species

•

Anaerobic, gram-negative rod. 위장병, 장외 감염은 연조직염부터 bacteremia, 사망까지 이른다.

(1) Vibrio parahaemolyticus

•

Extraintestinal wound infection은 chronic liver disease나 면역저하자에게서 흔하다.

(2) Vibrio vulnificus (★)

전세계적으로 감염을 일으키는 병원균

3가지 유형; 인간에게 병원성, 어류에 병원성, 민물에서 물고기와 접촉한 후 연조직 감염 및 패혈증을 유발하는 유형

40대 이상 남성, chronic liver disease, ESRD, immunosuppressant가 위험인자.

감염은 증상이 시작된 후 빠르게 나타난다.

Malaise, fever, vomiting, chills, diarrhea, skin vesiculation à necrotizing dermatitis and fasciitis à bacteremia, sepsis à 괴사조직의 제거나 절단이 필요할 수 있고 사망할 수 있다. (사망 최대 50%)

(3) Vibrio mimicus

Ear infection의 가능성

(4) Vibrio alginolyticus

Ear infection, soft tissue infection, sinusitis

항생제 내성이 흔하다.

(5) Photobacterium damsel

Vibrio-like wound infection.

근육괴사, 근막염, 패혈증, 사망 등

(6) Vibrio Cholerae

심각한 가끔 soft tissue infection도 일으킨다.

(7) Growth in Culture

해수의 bacteria이므로 특정 salt가 추가된 media에서 culture를 시도하는 것이 좋을 수 있다.

(8) Antibiotic Therapy

경미한 찰과상, 열상 등은 예방적 항생제가 필요하지 않다. 만성질환자, 면역저하자, 심각한 간질환이 있는 경우, PO ciprofloxacin, TMX-SMP, tetracycline 또는 doxycycline이 필요할 수 있다.

Streptococcus species 역시 연조직 감염을 자주 일으키므로 같이 커버할 수 있도록 penicillin 계를 같이 사용하는 것이 좋다.

Tintinalli’s : Doxycycline 100mg IV twice a day + 4 cephalosporin + surgical debridement

D3. Freshwater Bacteriology

(1) Diversity of organism

담수 환경도 다양한 pathogen이 있어 바다만큼이나 위험하다.

(2) Wound infections Caused by Aeromonas Species

연조직염 (특히 puncture wound), 위장관 감염

Fever, hypotension, jaundice, 심한 경우 vibrio-like sepsis가 되기도 함.

Tintinalli’s : Ciprofloxacin (500mg PO, twice a day) 이 효과적. Severe infection의 경우 antipseudomonal cephalosporin + surgical debridement

(3) Infection Caused by a Fish Pathogen, Streptococcus iniae

양식 어류의 표면에  iniae가 서식하고 있고, 양식 어류의 지느러미, 뼈, 어획도구 등에 찔려서 감염이 발생

Lymphangitis, cellulitis (주로 괴사는 없는), endocarditis, arthritis, encephalitis

일반적인 항생제로 잘 치료된다.

(4) Infection Caused by Desmodesmus armatus

엽록소를 가진 fungus, case report 수준으로 드물다. 외과적 괴사조직 절제로 치료하였으며, antifungal agent 투여 없었다.

(5) A General Approach to Antibiotic Therapy

만성질환, 면역장애, 특히 간 질환이 있는 경우에는 즉시 경구 ciprofloxacin (2번째 선택 TMX-SMP)을 투여한다.

감염이 발생하면 staphyllococci나 streptococci에 대해서도 고려해야한다.

Surgical debridement를 고려해야 한다.

E1. Shark

Treatment

Immediate threat to like is Hypovolemic shock.

All means available must be used to ligate large, disrupted arteries or to apply compression dressings.

Massive transfusion, fluid resuscitation

Tetanus vaccination, immunoglobulin

Surgical intervention

Dirty-contaminated wound

Intraoperative x-ray : one or more shark teeth

Shark phobia : 생존자들에게서 나타날 수 있다.

Prevention

상어의 서식지를 피하라.

피 (생리혈 포함), 특히 물고기의 피는 상어를 유인할 수 있다.

상어의 먹이가 되는 물고기떼와 같이 수영하지 마라.

상어를 놀리거나 궁지에 몰아넣지 마라.

물을 튀기거나 소동을 일으키지 마라.

상어를 마주치고 피할 수 없을 때엔 주둥이에 둔탁한 타격을 주거나 눈이나 아가미를 공격하라.

E2. Barracuda (꼬치고기)

•

Clinical aspect : 주로 bites

•

Treatment : shark의 그것과 동일하다.

E3. Moray Eels (곰치)

Clinical aspect : 주로 bites

Treatment

shark의 그것과 동일하다. Eel이 victim에 붙어 있는 경우가 있는데 decapitation해서 jaw의 압박을 풀어줘야 한다.

Infection의 risk가 매우 크다. 적절한 광범위 항생제를 쓰지 않으면, infection 치료에 수년이 걸리기도 한다.

E4. Giant Groupers (대왕바리)

: 큰 농어. 물린 상처가 많고, 상어의 그것과 동일하게 치료한다.

E5. Sea Lions and Seals

•

온화한 편이나 번식기엔 공격적이다. 요즘엔 동물원 사육사들이 많이 물린다. 

•

Cephalosporin이 큰 효과가 없으며, ciprofloxacin이나 tetracycline을 사용하는 것이 좋다.

E6. Needlefish (동갈치)

튀어 올라 날아서 관통상. 표면상처가 작아도 내부 상처가 클 수 있으며, foreign body에 유의해야 한다.

E6. Large Leaping Fish

튀어 올라 날아서 관통상이나

E7. Killer Whales

지능이 높으며, 장난끼가 많다. 바다사자로 착각한 인간은 좋은 먹이가 되기도 한다.

Giant Clams : 큰 조개. 물렸다고들 말은 하지만 report는 없다.

Giant Squid : 인간을 공격할 가능성이 있어 보이지만 아직 보고는 없다.

Giant Octopus : 잠수부의 마스크를 벗기거나 산소조절기를 건드리는 경우가 있었다.

Giant Manta Ray : 쥐가오리. 피부가 거친 편이라 잠수부들이 찰과상을 입기도 한다.

Mantis Shrimp : 사마귀 새우. 앞발을 매우 빠른 속도로 내리쳐 잠수부의 안면 마스크를 깨뜨리는 경우가 있었다.

Piranha : 주로 발뒤꿈치를 자주 문다. 심하면 발가락이 잘릴 수도 있다.

Snapping Turtle : 건드리면 무는 경우가 있다.

Triggerfish : 손가락을 물기도 한다.

Sawfishes : 특이한 길쭉하고 이빨이 있는 주둥이가 위협적일 수 있다.

Stony Corals

Clinical Aspect : Coral cut. 찌르는 듯한 통증, 홍반, 가려움증. Cellulitis로 진행될 수 있다.

Treatment : 즉각적으로 비누와 물로 세척하여 이물질을 제거해야한다. Suture를 해야 하면 delayed suture를 해야 한다. Topical antibiotics 가 도움이 될 수 있다. 예방적 항생제의 사용이 도움이 되며, 호전이 없을 경우 steroid를 사용한다. 보통 4~15주에 걸쳐 호전된다.

Electric Fish and Rays :

Rays : 전압이 높지 않고, 충격이 낮은 전류가 흐르지만, 성인을 기절시켜 익사를 유발시킬 수 있다.

전기뱀장어 : 전압이 높아 (500~650V) 인간을 무력화시킬 수 있다.
