A. Introduction and Epidemiology

고온 다습한 환경에서 주로 발생, 더운 기후 지역은 생리적 순응으로 인해 발생률 감소

B. Pathophysiology

•

Mechanism of heat transfer : 복사, 전도, 대류, 증발

B1. Response to heat stress

•

Normal core temperature : 36°C~38°C

(1) Physiologic response

•

피부 혈관 확장, 땀 분비 증가, 열 생산 감소, behavioral heat control

•

HR 증가 : 피부 혈관 확장으로 인한 stroke volume 감소로부터 cardiac output을 보상하는 기전.

•

Cholinergic stimulation 증가 → 땀분비 증가

•

Medication
: Anticholinergics, diuretics, phenothiazine, β-blocker, CCB, Sympathomimetic agent
→ Heat removal mechanism을 방해

•

Acclimation
: 땀 분비 증가로 thermal set point 낮춤, Aldosterone & ADH 증가

B2. Models of heat injury

(1) Excessive heat → direct cell injury → inflammatory cytokine 분비 → vascular endothelium damage → coagulation cascade activation & DIC

(2) Classic heat injury : 고온다습한 환경 노출 (ex. 에어컨 없는 집의 독거노인)

(3) Exertional heat injury : 고온에서 작업이나 운동

(4) Confinement hyperpyrexia

: 밀폐된 고온 환경에 노출 (ex. 더운 자동차 안에 남겨진 아이)

C. Clinical Features and Treatment

C1. Heat edema (열부종)

(1) 정의 : 더운 환경에 노출된 후 수 일 내 발생하는 손,발, 발목의 mild swelling

(2) 원인 : 피부 혈관 확장 및 간질액 저류, Aldosterone 및 ADH 분비 증가.

(3) 치료 : self-limited

C2. Prickly Heat (땀띠)

(1) 정의

: 옷을 입었던 부위에 발생하는 pruritic, maculopapular, erythematous rash

(= lichen tropicus, miliaria rubra, heat rash).

(2) 원인 : 땀에 불어난 각질층에 의해 땀샘이 막혀 발생하는 급성 염증 반응.

(3) 치료 : Antihistamine, calamine lotion, topical steroid

C3. Heat Cramp (열경련)

(1) 정의 : Skeletal muscle의 painful, involuntary, spasmodic contraction.

(2) 원인

: 땀을 많이 흘린 뒤 물이나 hypotonic solution을 과량 섭취 → 근육에서 상대적으로 sodium, potassium, magnesium 결핍 → 칼슘 의존성 muscle relaxation (rhabdomyolysis로 진행하는 경우는 드뭄)

(3) 치료 : Fluid & salt replacement (PO or IV)

C4. Heat stress

(1) 정의 : Heat stress에 의한 volume depletion or sodium depletion.

(2) 증상

•

Headache, nausea, vomiting, dizziness, malaise, muscle cramps, tachycardia,
orthostatic hypotension, syncope

•

체온은 정상이거나 40°C 이하 정도로 상승

•

No CNS impairment

(3) 치료

•

더운 환경으로부터 격리

•

Volume & electrolyte replacement

•

2~30분간의 치료 후에도 반응이 없으면 중심 체온을 39°C까지 내림

C5. Heat stroke

(1) 정의

•

Hyperthermia ( >40°C) + altered mental status

•

이전 criteria였던 Anhidrosis(무한증)는 없는 경우도 많음

(2) 증상

•

Heat stress의 일반적 증상 
+ CNS abnormalities (ataxia, confusion, hallucination, decorticate & decerebrate posturing, seizure, coma)

(3) 치료

•

즉시 cooling + supportive care

•

Dantrolene 효과 없음

D. Diagnosis

History & clinical presentation, 다른 질환 배제

E. Treatment

E1. Prehospital care

•

더운 환경 제거

•

의식 변화가 있는 경우 glucose level 우선 확인.

•

옷을 벗기고 cooling 시작 : 물을 뿌리고 airflow 제공, 젖은 수건이나 얼음 대주기

•

저혈압이 있는 경우 N/S 1~2L bolus drip

E2. ED management

(1) Initial resuscitation

: IV fluid & urine output, BST, core temperature check

(2) Cooling Technique → 어떤 방법이 우월한지는 밝혀지지 않았다.

•

Goal : Core temperature 39°C까지, overshoot hypothermia 방지.

•

Evaporative cooling
: Makkah cooling unit - 15°C 정도의 찬 물을 뿌리고 warm air (45°C)적용
: Shivering - short acting benzodiazepine (1차), phenothiazine (2차)
(Phenothiazine : Lower seizure threshold and cause hypotention)

•

Immersion cooling : Ice water tub에 환자 담그기

•

Invasive cooling measure : Cardiopulmonary bypass
cold water로 gastric/bladder/rectal lavage

•

Other cooling measure (단독 사용은 효과가 떨어짐)
: Cooling blanket, IV cold fluid, ice pack

F. Complication

F1. Hypotension

•

CVP 12~14mmHg & cooling method 시작 후 dopamine이나 dobutamine 사용 가능

F2. Electrolyte abnormality

•

Hyponatremia, hypernatremia, hypokalemia, hyperkalemia

F3. Hematologic disorder

•

Conjunctival / GI / pulmonary / renal hemorrhage,

•

Purpura, petechiae

•

Thrombocytopenia, hypoprothrombinemia, DIC 등.

F4. Acute renal failure, rhabdomyolysis

F5. Seizure

•

Cooling 시 특히 잘 발생하며, benzodiazepine 사용.

G. Disposition & Follow-up

•

Minor heat emergency (heat edema, heat cramps, heat stress)
⇒ Discharge & OPD f/u

•

Minor heat emergency with underlying disease (CHF, CRF) 
or severe electrolyte imbalance & Heat stroke : Admission

H. Prevention

1. 더운 장소, 시간, 직사광선 피하기, 그늘막 이용 2. 가볍고 헐거운 옷 착용 3. 탄수화물 섭취 늘리고 단백질 섭취 줄이기 (endogenous heat 생산 감소) 4. 충분한 수분 섭취 및 음주 금지 5. 수분과 함께 소금 섭취
